Prescription pick-up, assistance with making doctors appointments and preparing the home for care post-hospitalization

Recognize changes that may occur with the elderly patient, report and record information relevant to care and assess vital signs

Bathing, dressing, incontinence care/toileting, and transfer/mobility assistance

Managing behaviors, encouraging engagement, assisting with activities of daily living and keeping seniors safe in the home
